POSITION: Case Manager/Developmental Specialist
CLASSIFICATION: Hourly/Non-Exempt
INTRODUCTION: The Case Manager evaluates consumers in assigned caseload, develops and implements individual service plans, provides for consumer service needs and program objectives and acts as a resource for consumers, families, team members and program staff. Duties will also include direct supervision and training of consumers and participation in their program activities, within community settings or within the facility, behavioral plan implementation, direct care responsibilities and others.

Develop the appropriate need assessments and goals of Individual Support Plans and methods for implementation requested by consumer; support consumers’ choices in program planning. Participate and negotiate in case conferences and support planning. Act as a resource to consumers, families, referral and residential staff. Maintain positive relationship with referral agencies. |

Assist in-take and orientation process for new participants. Evaluate consumers to determine service needs with emphasis on preferences expressed by consumer. |

Ensure implementation of consumers’ ISP objectives & corresponding data collection. Document progress & needs. Train program staff re: DHSP implementation. Reinforce a team approach with open communication, support and instruction. |

Counsel consumers/make referrals and provide follow-up for additional services as indicated by consumer needs. Maintain knowledge of community resources. |

Assist in teaching new skills to consumers; facilitate individual or group activities in areas of self advocacy, education, recreation, social/developmental skill training, community integration & others determined by DHSP. Maintain knowledge of consumers’ learning styles, strengths & needs to teach skills effectively. Assist program staff in carrying out personal care duties for consumers, including, but not limited to transfers from wheelchairs, toileting, showering, lifting and transfers to and from vehicles. |

Participate in the development of Community Inclusion activities for rec., education & other areas, as indicated by therapeutic services |

Crisis intervention including: consistently carrying out behavior plans; intervening in crisis situations, as needed; proper implementation of Behavior Management & Emergency Restraint interventions; accurate & timely completion of incident reports & other related paperwork. |

Be aware of Human Rights & Sexuality policies, & monitor that consumers’ rights are upheld; act as a role model by displaying proper dress, language & a positive attitude. |

Ensure that Health & Safety policies & procedures are implemented; be familiar with evacuation routes, ensure safe & efficient evacuations during drills & actual emergencies; be aware of fire safety & prevention procedures; keep aisles & means of egress clean & accessible; report unsafe situations; monitor consumers during lunch, breaks & cab duty; & safely transport consumers to community sites. Keep program clean & organized by performing light housekeeping;. Always use safe lifting & transferring techniques. |

Organize and update consumer records; complete required paperwork in a timely and accurate manner. |

Participate in Quality Assurance and Case Records Committees’ survey processes; and contribute to the Program Evaluation Process, as requested. |

Qualifications: Bachelor’s Degree in Behavioral Sciences, Psychology, Rehabilitation Education, or related field with two years experience in Human Services; or Masters Degree in
Human Services and one year experience in the Rehabilitation field
Other Qualifications: First Aid and CPR Certification, Knowledge and use of behavior modification techniques,
Community Inclusion, and Principles of Normalization,experience in providing services to
individuals with Mental Retardation, Developmental Disabilities, and elderly individuals with
developmental disabilities. Strong writing skills.
