POSITION:                              Paraprofessional/Direct Care                 

 

CLASSIFICATION:                 Hourly/Non-Exempt

 

INTRODUCTION:  The paraprofessional implements individual service plans, acts as a resource for members, family and team members, and maintains responsibility for the direct supervision and training of members within the assigned program, and within community settings.

 

PRIMARY DUTIES

My initials indicate that I understand and accept these duties as assigned.

 

Ensure the implementation of member’s DHSP objectives and corresponding data collection; accurately document progress and needs related to goals.

 

Instruct, counsel and supervise members in: education, daily living skills, personal care, socialization, behavioral skills, self advocacy, and other areas identified in the DHSP.

 

Assist members to achieve goals by maintaining knowledge of their learning styles, strengths and needs and ensure that skills are taught accordingly; notify the Administrator, Program Director or Developmental Specialist’s of problems or concerns; maintain a normalizing, learning environment.

 

Communicate effectively and work cooperatively with supervisors including; Administrator, Program Director, Health Services Coordinator, Therapeutic Services (OT, PT, Speech and Behavior) and all members.

 

Maintain an attitude of professionalism at all times, in all environments and with all related persons to the program (i.e. members, families, supervisor, management, outside agencies and community members).

 

Be aware of Human Rights, DPPC Reporting Procedures, and Sexuality Policies and monitor that members’ rights are upheld; act as a role model by displaying proper dress, language, safe work practices and a positive work attitude.

 

Ensure that Health and Safety policies and procedures are implemented: be familiar with evacuation routes, ensure safe and efficient evacuations during drills and actual emergencies, be aware of fire alarm pull stations and their proper use, keep aisles and means of egress clean and accessible, report unsafe situations, monitor consumers at lunch, breaks, safely transport consumers to community sites. Keep program areas clean and organized, perform light housekeeping and train consumers to keep program areas orderly.  Always uses safe lifting & transferring techniques.

 

Carry out personal care duties for members, including, but not limited to transfers from wheelchairs, toileting, showering, lifting and transfers to and from vehicles.

 

Support members within Community Inclusion activities as recommended through the member’s DHSP for education, etc.

 

Crisis intervention including: consistently carrying out behavior plans; intervening in crisis situations, as needed; proper implementation of Behavior Management and Emergency Restraint Training; accurate and timely completion of incident reports and other related paperwork.

 

Works required days & hours. Reports to work on time.

 

Attend all meetings as advised by Day Habilitation Management.

 

Maintain all licenses, certifications and up to date trainings in all mandated areas (i.e. First Aid, CPR, SOLVE etc).

 

 

SUPERVISION RECEIVED: Administrator, Program Director, Health Services Coordinator, Developmental                                                     Specialists

 

SUPERVISION GIVEN:        Members.

                                                           

 

 

 

QUALIFICATIONS:                Bachelor’s Degree in Human Services or related field preferred. Related experience in Human Services and Behavior Management can be substituted for educational requirement.

 

Other Qualifications:                  SOLVE, First Aid and CPR Certification. Knowledge and use of behavior modification techniques and Community Inclusion.
