40 hrs/week

The FC RN Care Coordinator employs an anticipatory, proactive approach to case management to ensure that FC patients receive a  quality of care in a cost-effective manner.
Specifically, (she/ he):
Organizes, coordinates, monitors, evaluates and modifies plans of care for FC patients requiring case management.
Focuses on selected complex medical and psychosocial needs of FC patients and their families, individualizing standards of practice and care to meet there needs.
Proactively facilitates and coordinates a plan of care with other members of the health care team, including but not limited to PCPs, Specialists, Mid Level Practitioners, FCHP Case Managers, Social Service.

Graduate from an accredited school of Nursing; Current licensure as a Registered Nurse in Massachusetts; current driver’s license.  Certified Case Manager preferred.
A minimum of three to five years clinical experience as a Registered Nurse in an inpatient setting with a minimum of two years of clinical experience as an outpatient care manager or home care setting.
