Harbor Medical Associates is a group of physicians, nurse practitioners, physician assistants, nurses and support staff dedicated to providing a broad spectrum of primary care services to patients in over ten South Shore communities. We are committed to the ideal that patient care comes first.
Harbor Medical Associates seeks a Nurse Practitioner to practice at our Weymouth Internal Medicine Practice (1221 Main St.).
ESSENTIAL FUNCTIONS:
- Performs routine health exams by obtaining baseline histories, review of symptoms and doing physical exams.
- Differentiates normal findings from abnormal.
- Screens and refers patients to supervising physician or to appropriate health care specialists.
- Orders and interprets appropriate laboratory work.
- Shares responsibilities and collaborates in the care of a selected panel of patients with a physician who supervises medical management.
- Develops and implements a comprehensive health plan for patients and families. This includes assessment of patients social and emotional needs as well as medical management.
- Communicates relevant patient data to the physician verbally and in the medical record using a problem oriented approach.
- Identifies and manages specified acute illnesses within their scope of practice.
- Monitors and manages stabilized patients with chronic diseases such as hypertension, diabetes, obesity according to agreed upon protocols.
- Participates in review of quality of care and works toward appropriate productivity.
- Shares responsibility with the physicians for prescription refills, specialty -referrals, communicating test results to the patient and general management and coordination of care.
ENVIRONMENTAL/WORKING CONDITIONS:
Work is performed in a healthcare environment and involves frequent contact with patients. Work may be demanding at times. Interaction with others is constant, and can be interruptive of other tasks being worked on.

REQUIREMENTS:
Three to five years experience mandatory; experience with geriatric population preferable. Graduate of an accredited school of nursing or an accredited program for Physicians Assistants. Current authorization to practice as a nurse practitioner or physician assistant by the Board of Registration.
