In this role, qualified candidates will be responsible for the following:
1. Provides direct care, counseling and teaching to a select patient population. Incorporates the concepts of health maintenance, prevention and promotion into daily practice through patient education and counseling. 2. Performs physical examinations and provides preventive health measures appropriate to patient needs. Screens for substance abuse, tobacco use, domestic violence, sea belt use, immunization status, nutrition, exercise, history of falls as necessary and initiates appropriate interventions. 3. Orders, interprets and evaluates appropriate lab and diagnostic tests to assess patient clinical problems and health care needs. 4. Arranges for appropriate plan of care and follow-up based on outcome of diagnostic, lab and physical assessment findings. 5. Orders medications and writes prescriptions according to organizational and regulatory policies and procedures. 6. Records pertinent patient care findings in the electronic medical record in a timely manner. Completes forms and reports for insurance companies, VNAs, and other agencies, as needed. 7. Sets priorities for appropriate and efficient management of patient care that reflects cost effective management of time, available human resources, supplies and equipment. 8. Maintains interdisciplinary approach to planning and communicating patient care information by discussing patient data with appropriate physicians and other health care professionals. Demonstrates awareness of community resources and assists staff, patients and families to effectively use them. 9. Collaborates effectively with supervising physician. 10. Actively participate in quality assurance/quality improvement activities at CHA; in peer review activities; in pay-for-performance initiatives, electronic medical records initiatives and other similar initiatives of Employer or the Health Plans. 11. Performs other related duties as assigned or directed. To qualify, candidates must possess the following: -Graduate of a Physician Assistant program approved by the Commission on Accreditation of Allied Health Education Programs (CAAHEP). -Massachusetts PA license, Massachusetts controlled substance license, and federal DEA license. -Must have taken and passed the National Commission for Certification of Physician Assistants (NCCPA) examination. -At least 2 years of clinical experience as a Physician Assistant in primary care is preferred.
